A self-assessment is a great tool to share with your clients at any time during your coaching program as a way to measure their growth, progress, or transformation.


Go to My Forms using a menu in the top right corner.



Click on "Create New Form" button:



Choose "Self-Assessment" to get started.




Name your form and enter the description or introduction text.



Now you can start adding your form questions.


All you need to do is enter the question and Coaching Genie will automatically display a 1-5 scale for your clients to choose their answer.


After entering all questions, it's time to create the "scoring key" - an interpretation of what the score means.



Understanding the Scoring


You can have 3, 4, or 5 scoring interpretations. 


Depending on which number you choose, the system will automatically calculate the score based on your clients' answers, and display the matching interpretation. 


For example, below we selected 3 scoring interpretations. So if your clients score 0%-33%, they will see the description of the result in the first box. If they score 34%-66%, they'll see the description of the result you enter in the second box. And so on.



Digging Deeper: The total possible score equals "number of questions X 5". For example, if you have 10 questions, the total possible score is 50 points, which equals 100%. If your client chooses 4 as the answer for each question, their total score is 10 X 4 = 40 points, which equals to 80%. Therefore your client will see the scoring interpretation you enter in the 3rd description box.




Saving Your Self-Assessment


When you're finished adding the questions and the scoring key, you can choose to either display the result of the form (answers they submitted), or redirect them to another page of your choice. 


You can also choose to allow clients to edit their answers later:



When finished, remember to hit "Save". 


You can update your form any time by going into the form, making the changes you want, then clicking "Update".
